About The Position

We are seeking a detail-oriented and knowledgeable MDS Coordinator (RN or LPN) to join our Skilled Nursing Facility team in Oregon. The MDS Coordinator is responsible for accurately completing and managing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations and facility policies. This key role helps optimize resident care planning, quality outcomes, and reimbursement accuracy through timely assessments and interdisciplinary coordination. Requirements

  • Valid Oregon RN or LPN license in good standing.
  • Strong knowledge of RAI process, PDPM, care planning, and federal/state reimbursement guidelines.
  • Excellent communication, critical thinking, and organizational skills.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.

Nice To Haves

  • Prior experience as an MDS Coordinator or in a similar role in a SNF setting.
  • Experience with electronic health records (EHR); familiarity with software such as PointClickCare is a plus.

Responsibilities

  • Coordinate and oversee the timely completion of MDS assessments and care plans in accordance with CMS guidelines and Oregon SNF regulations.
  • Ensure accurate and complete documentation to support RUG/PDPM classifications and reimbursement.
  • Collaborate with interdisciplinary team (IDT) to develop individualized care plans that reflect resident goals and clinical needs.
  • Monitor changes in resident status and ensure MDS updates and significant change assessments are initiated as needed.
  • Audit and validate MDS coding accuracy; provide education to nursing and therapy staff as needed.
  • Serve as a resource for survey readiness, quality measures, and QAPI initiatives.
  • Participate in resident care conferences and ensure care plans are effectively implemented and updated.
