An MDS Coordinator (Minimum Data Set Coordinator) is a highly skilled nursing professional responsible for overseeing the comprehensive assessment process for residents in long-term care facilities. Requirements

  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
  • Current state license as an RN
  • Proof of current CPR certification
  • Compassionate, friendly demeanor
  • Excellent communication skills
  • Must be able to pass a background check and fingerprint process

  • RAC-CT(REsidnet Assessment Coordinator Certified) preferred but willing to train the right candidate.

Responsibilities

  • Oversee MDS Assessments : Coordinate and ensure the timely and accurate completion of the Minimum Data Set (MDS) for all residents, maintaining compliance with applicable standards.
  • Interdisciplinary Collaboration : Partner effectively with nursing staff, physical and occupational therapists, dietitians, and attending physicians to collect comprehensive data and develop holistic, person-centered care plans.
  • Care Planning : Utilize MDS data to formulate individualized care plans that respectfully address each resident's unique strengths, needs, and preferences.
  • Reimbursement & Compliance : Ensure all documentation complies with federal regulations, including OBRA guidelines, to facilitate accurate Medicare and Medicaid reimbursements, incorporating RUGs classifications and facility quality measures.
