About The Position

The primary purpose of the MDS Coordinator position is to conduct and complete the MDS 3.0 assessment of the residents of the facility in accordance with the State and Federal requirements. It is imperative that the MDS Coordinator is kept abreast of all new admissions, discharges, hospital transfers and resident medical changes. As the manager of the resident's MDS this information is essential.

Requirements

you must be an RN with MDS experience to qualify for this position
High school diploma or general education degree (GED) and one to three months related experience and/or training; or equivalent combination of education and experience.
Completion of an approved nursing program.
Attends and participates in continuing education programs
Enter, adjust and retrieve data using a microcomputer or personal computer and keyboard.
Must be able to read, write, speak and understand the English language.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of residents or employees of organization.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Ability to deal with problems involving several concrete variables in standardized situations.
Must possess a current, unencumbered, active license to practice as a registered nurse in Maryland
The employee is regularly required to sit.
The employee frequently is required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, or crawl; and talk or hear.
The employee must occasionally lift and/or move up to 50 pounds independently or move up to 100 pounds to a minimum height of 4 feet and be able to push, pull, move and/or carry such a weight a minimum distance of 100 feet with assistance from another or mechanical lift Specific vision abilities required by this job include close vision, distance vision, peripheral vision, and depth perception.

Responsibilities

Conduct and coordinate the development and completion of the resident assessment (MDS) 3.0 in accordance with current rules, regulations and guidelines that govern the residents assessment, including the implementation of CAAs and Triggers.
Review, sign and date all forms within proper time frame.
Calculate Triggers and CAAs and work with Patient Care Coordinators in performing the CAA protocols.
Work with Admissions Coordinator in developing and maintaining the MDS schedule calendar, establishing appropriate reference dates for assessments. Distribute schedules to other member of the interdisciplinary care plan team.
Coordinate and manage the weekly care plan meetings.
Participate in facility surveys made by authorized government agencies.
Conduct or coordinate the interviewing of each resident for the assessment. Each resident is assessed upon admission, quarterly, annually, for significant changes and for others reasons as required.
Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment. Assist the interdisciplinary team and determine when a significant change has occurred that requires an assessment to be performed.
Ensure that all appropriate health professionals are involved in the assessment; communicate with nursing staff and other care plan team members (Dietitian, Social Worker, Activity Staff) in order to obtain accurate and timely data.
Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties for false certification.
Assist in developing the written plan of care based upon the data on the MDS and CAAs for each resident that identifies the problems/needs of the resident, indicates that care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
Ensure that quarterly and annual resident assessments are ready for transmission to the state on timely basis.
Sign and date the assessment instrument to certify its completion.
Performs initial MMDS on new admissions.
Understand and observes all other resident's rights as indicated in the employee handbook.
Maintain an effective, friendly working relationship with health professional, physicians, consultants and governmental agencies that may be involved in the resident assessment functions of the facility.
Report equipment malfunctions to appropriate staff and complete work orders when indicated.
Identify and remove safety hazards/devices and report to appropriate personnel.
Assure a safe, clean, and clutter free environment.
Utilize standard precautions by wearing protective equipment (goggles, gowns, gloves) when indicated, and uses correct hand washing technique.
Follow facility fire and safety policies.
Attend and participate in facility meetings and committees as assigned.
Serve as a positive role model to others.
Attend all mandatory and other scheduled in services.
Adhere to rules as outlined in the employee handbook.
