About The Position

At our behavioral health center (Crestwood Terrace, an Independent Care Facility or ICF), the MDS Nurse plays a vital role in supporting regulatory compliance while ensuring each resident's clinical and behavioral health needs are accurately reflected. Your assessments help guide care planning, program services, and reimbursement, all within a recovery-focused, therapeutic environment. Requirements

  • Registered Nurse (RN) license in Illinois
  • Minimum of 1+ years of MDS experience in a Skilled Nursing Facility or ICF

Nice To Haves

  • Current MDS 3.0 certification is preferred

Responsibilities

  • Complete, coordinate, and transmit MDS assessments in accordance with ICF regulations and required timelines
  • Ensure clinical documentation supports resident needs, services provided, and regulatory standards
  • Partner with nursing leadership, providers, and interdisciplinary teams to maintain compliance and accuracy
  • Track changes in resident status and update assessments as required
