About The Position

Webster at Rye, a non-profit facility located in coastal Rye, NH, is looking to hire an MDS Coordinator to join our team! This exciting role is a rare opportunity and a perfect fit for an RN/LPN who possesses strong organization and communication skills. This role is responsible for gathering information on our current and future residents for assessment including physical and mental states. The MDS Coordinator assess charts and communicates with health care teams to create applicable health care plans for current and incoming residents at our gorgeous property located at Webster at Rye. Requirements

  • Registered Nurse/LPN in good standing and currently licensed by the State of NH.
  • Previous experience in a hospital, long-term facility, or other healthcare related facility is required.
  • Prior experience completing MDS’s, and Care Plans is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care; AccuMed and previous experience as an MDS Coordinator is preferred.
  • Deep medical terminology knowledge
  • Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory.
  • Excellent leadership skills
  • Superb decision-making skills

Nice To Haves

  • AccuMed and previous experience as an MDS Coordinator is preferred.

Responsibilities

  • Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
  • Determine potential Resource Utilization Groups (RUGs) and expense associated with a potential admission.
  • Review all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator.
  • Complete and assure the accuracy of the MDS process for all residents.
  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
  • Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement.
  • Facilitate problem-solving for complicated admissions.
  • Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
