About The Position

Coordinates the timely and accurate completion of the MDS process from admission to discharge, ensuring compliance with federal and state regulations.

Requirements

Graduate of Nursing or Practical Nursing school.
Maintains current Florida RN or LPN license renewed every two years.
Current AANAC (RAC-CT) certification or is willing to obtain.
Maintains current BLS certification.
Minimum of one (1) year MDS experience in a sub-acute and long-term care facility.

Responsibilities

Assists with the coordination and direction of care management meetings, including the review of resident care and the setting of assessment reference dates.
Collaborates with the interdisciplinary team to assess resident needs and develop a comprehensive plan of care.
Updates, maintains, and monitors the written plan of care to ensure that each resident’s identified needs or approaches have appropriate plans in place.
Monitors MDS and care plan documentation for all residents to ensure that the medical record contains documentation supporting MDS coding and sequencing.
Supports the tracking system of MDS schedules to include time frames, due dates, and timely completion and submission of assessments.
Attends department meetings, shift huddles, and other relevant meetings to gather information, communicate changes, and maintain accurate records.
Provides in-service training and education on ADL coding and skilled documentation.
Ensures complete and accurate resident information is documented in the appropriate section of the electronic medical record.
Completes accurate coding of the MDS with information obtained from the medical record, record reviews, direct observation, and conducting interviews with the facility staff, resident, and family members as required.
Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state, and local standards.
Maintains compliance with all Orlando Health policies and procedures.
