About The Position

Regional MDS Coordinator (RN) – Skilled Nursing | 3542043 Location: New York (Oversight of 5 Skilled Nursing Facilities) A multi-site Skilled Nursing operator in New York is seeking an experienced Regional MDS Coordinator (RN) to oversee and standardize MDS operations across five facilities. This is a permanent, senior-level leadership role requiring extensive Skilled Nursing and multi-site MDS experience. The Regional MDS Coordinator will directly impact PDPM performance, Case Mix Index (CMI), regulatory compliance, and revenue integrity across the organization. Position Overview The Regional MDS Coordinator will establish, audit, and optimize MDS processes across all five facilities to ensure consistency, reimbursement accuracy, and survey readiness. This role works closely with Corporate Clinical Leadership and Facility Administrators to drive documentation integrity, regulatory compliance, and operational performance. This is a revenue-critical and compliance-driven position. The Regional MDS Coordinator plays a central role in protecting reimbursement integrity and maintaining regulatory consistency across the organization. Requirements

  • Active New York RN license required
  • Minimum 3+ years of MDS experience in Skilled Nursing Facilities
  • Demonstrated experience overseeing or supporting MDS operations across multiple facilities strongly preferred
  • Deep understanding of PDPM reimbursement drivers and CMI optimization
  • Strong working knowledge of CMS regulations and NY DOH survey processes
  • Experience conducting internal MDS audits and compliance reviews
  • Hospital-only experience will not be considered

Responsibilities

  • Oversee and standardize MDS processes across five SNF locations
  • Audit MDS assessments for accuracy, timeliness, and reimbursement optimization
  • Monitor PDPM components and Case Mix Index performance
  • Identify documentation gaps and implement corrective action plans
  • Support facilities in preparation for survey activity and regulatory review
  • Train, mentor, and support facility-based MDS Coordinators
  • Collaborate with regional and facility leadership to strengthen compliance and revenue stability
