About The Position

At our behavioral health center (Frankfort Terrace, an Independent Care Facility or ICF), the MDS Nurse plays a vital role in supporting regulatory compliance while ensuring each resident’s clinical and behavioral health needs are accurately reflected. Your assessments help guide care planning, program services, and reimbursement, all within a recovery-focused, therapeutic environment. Requirements

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Illinois
  • Minimum of 1+ years of MDS experience in a Skilled Nursing Facility

Nice To Haves

  • Current MDS 3.0 certification is ideal, but not required
