The LTC MDS Coordinator RN Supervisor leads and manages the MDS and care planning process to ensure high-quality, compliant care for long-term care residents. This role coordinates interdisciplinary teams, oversees assessments, and ensures accurate documentation in alignment with CMS and state regulations. What You’ll Do Complete and oversee comprehensive MDS assessments, including PDPM scheduling and all required screenings Develop and maintain individualized care plans with measurable goals Coordinate with interdisciplinary teams to ensure timely and accurate completion of MDS/CAAs and care plans Lead and facilitate care plan and Medicare meetings, including scheduling and communication Maintain accurate documentation, charting, and submission of MDS data to state agencies Monitor changes in resident condition (weight, falls, medications, infections) and communicate concerns Support discharge planning and continuity of care Participate in leadership duties including staffing, performance management, and team support Ensure compliance with safety standards, regulatory requirements, and quality improvement initiatives Provide supervision and direct care support as needed, including on-call responsibilities
