About The Position

We are looking for a qualified MDS Coordinator to join our team at our new upcoming skilled nursing facility, located in Madison, Ohio! Opening Spring 2026! Arcola Creek Health & Rehabilitation, a BRAND-NEW skilled nursing facility located in Madison, OH is looking for a MDS Coordinator with passion and purpose to join our dedicated management team! This position is responsible for the following: Ensure compliance with the clinically based reimbursement procedures Ensure that the facility is compliant with the guidelines for the Minimum Data Set (MDS), Quality Measures, Medicaid and Medicare RUGs, Medicaid Case Mix, and Medicare’s skilled level of care criteria including the supportive documentation requirements. Monitor and ensure MDS assessments are accurately coded to present a complete picture of residents' clinical status and services rendered Identify opportunities for improvement and best practices Ensure that MDS Assessment procedures are followed The successful candidate will possess, at minimum, the following qualifications: Licensed or Registered Nurse in State of Ohio. 1+ years of experience as a MDS or equivalent experience Point Click Care experience a plus Why Work with Us?
