Requirements

  • To perform this job successfully, an individual must be proficient in the Microsoft Suite products.
  • Must maintain all required continuing education/licensing.
  • Must remain in good standing with the State Board of Nursing at all times.

Responsibilities

  • Provide oversight for the facility’s weekly PDPM meetings for skilled patients.
  • Track and monitor the effective utilization of PDPM tools (including but not limited to Physician Diagnosis Verification, Admission/Discharge CSR [GG], Weekly CSR, PDPM Weekly Tracking Tool, and Point Right).
  • Monitor QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations for optimal outcomes.
  • Provide PDPM and MDS training to new MDS Coordinators.
  • Assess and determine the health status and level of care for all new admissions.
  • Ensure the accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  • Communicate the level of care for new residents to all disciplines.
  • Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission according to regulatory time frames, ensuring thorough documentation as mandated by federal, state, and medical standards.
  • Maintain an accurate schedule of all MDS assessments, including proper reference dates throughout the resident’s stay.
  • Responsible for data entry to assure accurate data entry and electronic submission of MDS assessments.
  • Verify electronic submissions of MDS, performing corrections when necessary and maintaining appropriate records.
  • Schedule and conduct resident care conferences in compliance with state and federal regulations, ensuring completion of all MDS reviews prior to these conferences.
  • Assist disciplines in formulating and revising care plans, ensuring that residents’ present and potential problems are identified and prioritized, with realistic goals established and appropriate nursing interventions provided.
  • Evaluate resident care plans for comprehensiveness and individuality.
  • Assess the achievement or lack thereof of desired outcomes, ensuring that care plans are reassessed and revised appropriately.
  • Responsible for all level of care changes within the facility and notify all departments when such changes occur.
  • Generate appropriate forms to complete level of acuity changes and transmit these forms to the appropriate agency as required by state law.
  • Maintain the confidentiality of all resident care information, including protected health information.
