About The Position

City View Post Acute is seeking a skilled MDS Nurse (LVN) to oversee the facility’s weekly PDPM meetings for skilled patients and ensure effective utilization of PDPM tools. The role involves tracking and monitoring quality measures and performance metrics, providing data analysis and clinical recommendations, and delivering training to new MDS Coordinators.

Requirements

  • Graduation from an Accredited School of Nursing
  • Must maintain good standing with the State Board of Nursing and fulfill all continuing education and licensing requirements.
  • Proficiency in Microsoft Suite products
  • Ability to communicate tactfully with residents, families, staff, and regulatory agencies.
  • Independent decision-making and problem-solving skills.
  • Ability to handle physical demands of the job, including lifting up to 25 pounds.

Nice To Haves

  • Bachelor of Science Degree in Nursing preferred.
  • Minimum of 3 years of supervisory experience preferred.
  • Knowledge of PCC preferred.

Responsibilities

  • Assess and determine the health status and level of care for all new admissions.
  • Ensure the accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  • Communicate the level of care for new residents to all disciplines.
  • Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission according to regulatory time frames.
  • Ensure completeness and thoroughness of documentation as mandated by federal, state, and medical standards.
  • Maintain an accurate schedule for all MDS assessments, ensuring proper reference dates throughout the resident's stay.
  • Responsible for data entry, electronic submission of MDS assessments, verification, and maintaining records.
  • Schedule and conduct resident care conferences in compliance with regulations and complete MDS reviews prior to these conferences.
  • Assist in formulating and revising care plans and evaluate their effectiveness.
  • Ensure confidentiality of all resident care information, including protected health information.
  • Notify all departments of level of care changes and generate appropriate forms for processing as required by state law.
