Exciting per diem opportunity for an advanced practice provider to work in collaboration with Maternal-Fetal Medicine Faculty and APPs in the provision of clinical care for patients with obstetric needs. Specifically, the position would focus on providing Diabetes care during pregnancy to patients within the University of Rochester Health Care System and surrounding Finger Lakes Region, in addition to supporting the care of the high-risk obstetric patient populations cared for in our MFM clinics. This provider, as part of a multidisciplinary team, will provide direct patient care and management within our Diabetes in Pregnancy Program (DiPP). The position would also involve multidisciplinary care management of patients in all MFM practices, including coverage of MFM clinics at Red Creek, coverage of electronic/phone based patient questions, result and order follow-up, and regional care coordination. Candidates must have excellent communication skills, self-motivation for on-going learning, flexibility and teamwork experience. Underrepresented minorities are strongly encouraged to apply. The per diem position would comprise two-and-one-half days of patient facing time with one additional half day allotted to administrative efforts; this need could be divided over two candidates. Requirements

  • Candidates must have excellent communication skills, self-motivation for on-going learning, flexibility and teamwork experience.
  • Candidates must be NYS NP, PA or CNM licensed.
  • While obstetrical experience is required, other unique skills in the care of the high-risk perinatal patient would also be welcome.

Nice To Haves

  • Bilingual abilities would be a welcome addition to the practice but are not required.

Responsibilities

  • Leading our DiPP patient review meeting every Thursday at noon.
  • Reviewing glucose logs and managing oral and injectable medications, including My Chart messages, telemedicine/telephone visits with patients
  • Initiating and ongoing care for newly diagnosed diabetes in pregnancy (gestational diabetes), including diabetic education (in conjunction with our DiPP Registered Dietitian) as well as teaching and provision of supply scripts.
  • Initiating and ongoing care (including pump management) for patients with pre-gestational diabetes, including diabetic education as well as teaching and provision of supply scripts.
  • Performing postpartum follow up visits for DiPP patients
  • Managing in-basket messages, results, etc. for DiPP patients.
  • Postpartum assessment/pregnancy debriefing and coordination of long-term care.
  • Coverage of MFM clinics at Red Creek, coverage of electronic/phone based patient questions, result and order follow-up, and regional care coordination.
