Under direct and indirect supervision, the Care Coordinator will utilize multiple screening tools to determine MAT eligibility, provides general MAT education, and care coordination services which include referral and linkage to a community MAT provider and Davidson Medical Ministries (DMM). The responsibilities of the Care Coordinator include, but are not limited to, the following: Care Coordination Services Assess inmates within forty-eight (48) hours of the inmate being admitted into DCDC to determine MAT eligibility and provide general education regarding the MAT Program and DMM. To determine MAT eligibility, the Care Coordinator will complete a Social Determinants of Health (SDOH) Assessment, a Clinical Opiate Withdrawal Scale (COWS) Assessment and provide a Jail Intake Survey to each inmate. If appropriate, the Care Coordinator will refer the inmate to DCDC jail medical staff for initial Vivitrol dose. Coordinate with DCDC to ensure the Care Coordinator can meet with the inmate prior to release to discuss post release services. Assist with linkage and coordination to DMM for post-release medical services or another medical provider of inmate’s choice. Assist with linkage and coordination to Daymark for outpatient MAT or another MAT provider of inmate’s choice. Submit inmate’s information into the Daymark Electronic Health Record (EHR). Provide Jail Post Release Survey and complete an updated COWS at inmate’s release. This program is voluntary and must be agreed upon by the individual.
