Apply today! As the Master Social worker for Banner Home Health, you will see Home Health Patients. You will assist patients and families to identify resources in the community. Help patients access these resources. Assist patients and family with disease progression and understanding the disease process. Help patient and family identify supports system. Assist families with understanding care giver burnout. Help the patient with motivational interviewing to make informed decisions about their health care. You will work alongside a team of nurses, and therapist to help the patient regain prior level of functioning. LOCATION: West Valley Central Corridor along I-17 SCHEDULE: Training 60-90 day Working 3 8hour or 5 8hour shifts for faster training (Days can vary from week to week depending on class schedule.) 8:00am- 5:00pm Monday- Friday After Training PRN Position Call Outs/ PTO/ Leave Monday- Friday 8:00am-5:00pm Minimum of 2 shifts per month POSITION SUMMARY This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.
