This position is responsible for assessment and management of the palliative care consulted patient population with regards to complex grief, trauma and psychosocial needs of patients served by the inpatient Palliative Medicine Team at Banner University. The palliative care social worker will be the main point of contact for both palliative team members and patients through the acute care stay and with some limited follow up after discharge. The aim is provide social support to the patient and family system as a whole, to best align patient and family goals and values with supportive medical care, leading to reduced preventable re-admissions and improved patient centered outcomes. A strong applicant engages the appropriate resources within the multidisciplinary team and patient and family community to achieve optimal results for the patient, family, and care givers. This position provides comprehensive care coordination for patients as assigned and provides detailed resources as appropriate to individual family needs this includes some discharge planning and medical prior authorization requests. Ideally the candidate will possess a background in Healthcare/Palliative medicine (or hospice), working with physicians and administrators to execute strategies. Strong proficiency of Microsoft Products (Excel, Word, Outlook), will use Cerner and other internal dashboard systems. This position is Monday through Friday 8am to 4:30 pm. POSITION SUMMARY This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.
