About The Position

RiseWell Community Services is looking for a full-time Master Level Counselor – Family Specialist to join our Assertive Community Treatment Program in West Babylon, New York. In this role, you’ll be utilizing your own professional experience to provide tailored care and guidance to participants and families. Our Assertive Community Treatment teams provide a full suite of treatment to individuals dealing with serious mental illness, so they can get the support they truly need. ACT services aim to prevent relapse, teach valuable life skills, provide assistance, and connect individuals with essential community resources so they can move forward and thrive in work, school, home life, and social relationships. As a part of our ACT team, you’ll be working as a collaborate, mobile, multi-disciplinary treatment team, both in office and in the community. In this role, you will provide direct services with a focus on family services Work on a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness. Maintain case records for caseload assigned by the Team Leader. Schedule regular meetings with participants. Conduct group sessions as per staff discipline Collaborate with families and identify needs, strengths, goals, and resources to support families’ health and wellness. Establish, follow up, and maintain new referrals. Complete Needs Assessments. Complete weekly progress updates. Complete visits based on requirements from Team Leader. Prepare and submit reports by deadlines

Requirements

  • Master’s degree or higher in social work, psychology, mental health counseling, or marriage and family therapy.
  • Rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred).
  • A valid and clean NY State driver license.
  • Minimum 1 years of experience in a treatment or rehabilitation setting.
  • Fingerprinting, criminal record check.
  • Approval from NYS Office of Mental Health
