About The Position

RiseWell Community Services is looking for a full-time Master Level Counselor – Criminal Justice Liaison to join our Forensic Assertive Community Treatment Program in Queens, New York. In this role, you’ll be utilizing your own professional experience to provide tailored care and guidance to participants and families. Our Forensic Assertive Community Treatment teams provide a full suite of treatment to individuals dealing with serious mental illness, so they can get the support they truly need. FACT services aim to prevent relapse, teach valuable life skills, provide assistance, and connect individuals with essential community resources so they can move forward and thrive in work, school, home life, and social relationships. As a part of our FACT team, you’ll be working as a collaborate, mobile, multi-disciplinary treatment team, both in office and in the community. In this role, you will act as a member of a mobile, multi-disciplinary treatment team who integrates information and expertise on dual-recovery treatment.. Maintain case records for caseload assigned by the Team Leader. Schedule regular meetings with participants. Requirements

Master’s degree or higher in social work, psychology, mental health counseling, or marriage and family therapy. Rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred)
A valid and clean NY State driver license
Minimum 1 years of experience in a treatment or rehabilitation setting
Fingerprinting, criminal record check
Approval from NYS Office of Mental Health

Responsibilities
