About The Position

This full-time, non-exempt position serves a dual role focusing on both social services and census building for Wheat State Manor. The primary social services responsibilities include identifying psycho-social situations, establishing well-being goals, and acting as a liaison between residents, families, and community resources. This involves coordinating admissions, completing paperwork, assisting with payor eligibility (Medicare, Medicaid, Insurance), developing and implementing social care plans, scheduling care plan meetings, arranging ancillary services, monitoring psychotropic medications, and coordinating discharge plans. The role also involves educating families and staff on resident care and rights, and assisting with state surveys. The primary census building responsibilities involve developing and maintaining referral networks, managing leads, implementing marketing plans, representing the facility in the community through events and civic participation, developing advertising strategies, and maintaining social media presence. The position reports to the Administrator and requires a professional with strong interpersonal, organizational, and computer skills.

Requirements

  • LBSW, LMSW, or Social Service Designee required
  • Must be a self-starter and have excellent time management skills and be able to work within deadlines.
  • Excellent customer services skills to include ability to maintain a courteous, friendly, and helpful attitude with residents, visitors, and co-workers.
  • Ability to interact and build relationships with older adults and families.
  • Strong creativity and organizational skills as well as effective and professional communication skills required.
  • Computer skills with Microsoft Office (Word, Excel, and PowerPoint) and ability to learn and utilize Point Click Care (PCC).
  • Excellent oral and written communication and presentation skills.
  • Adherence to State Operation Manual (KDADS) and Power DMS Policy and Procedures.

Responsibilities

  • Identify psycho-social situations, establish goals for the highest practicable level of well-being, and serve as a liaison between residents, families, and community resources.
  • Participate in initial admission screenings, identifying needs, and meeting with family members.
  • Coordinate admission process and complete all facility admission paperwork with resident/DPOA/Family.
  • Answer questions, familiarize elder and family with life routines and ensure a smooth transition into the community.
  • Work with payors such as Medicare, Medicaid, and Insurance companies to determine eligibility and coverage available, and expedite application process and ensure required documentation is submitted timely to ensure payment.
  • Ensure Advance Directives are on file.
  • Complete CARE assessments/PASRR.
  • Follow up with resident and family after admission to address any concerns and gather additional information to assist with Medicaid application as necessary.
  • Assess and develop a plan of care that meets residents’ needs.
  • Develop and implement social care plans and resident assessments ensuring that social services components of the MDS/UDA’s are completed and signed on a timely basis.
  • Complete social history upon admission.
  • Schedule care plan meetings in coordination with interdisciplinary team and MDS timing.
  • Provide or arrange for social services for residents/family members as appropriate.
  • Assist in making referrals and arranging appointments and transportation with outside providers for ancillary care services.
  • Communicate with family and facility staff ensuring appointments occur within scheduled timeframes and in keeping with regulations.
  • Monitor resident behavior, effectiveness, and potential adverse effects of psychotropic medications with goal of reduction of psychotropic medications.
  • Assist in development of alternative therapeutic strategies to psychotropic medications.
  • Consult with or arrange for psychiatrist involvement if needed.
  • Coordinate and formulate interdisciplinary discharge plans for residents.
  • Assure successful transfer of residents both within the campus and to outside agencies within the community.
  • Assure leave of absences are processed smoothly, assure all aspects of admissions process of residents is done in compliance with state and federal regulations and in compliance with Wheat State Manor guidelines.
  • Complete all documents related to social work.
  • Educate families and residents regarding placement, care, and resident rights.
  • Provide education to staff on issues related to resident care, rights, and behaviors.
  • Establish team spirit between residents and employees.
  • Assist with the State Survey process.
  • Make evening or weekend appointments or home visits if necessary.
  • Perform additional duties as assigned.
  • Develop and maintain network with a variety of referral sources.
  • Control lead flow and ensure timely review and decision making by all appropriate team members to ensure highest possibility of receiving admission.
  • Interact with all leads whether provided through advertising, public relations, referral, or personal contact and turn those leads into residents of the community using professional selling skills.
  • Know procedures for lead tracking and keeping accurate records on all leads and prospects.
  • Create, implement, and update the approved census building plan.
  • Professionally represent location in the broader community.
  • Conduct and participate in cultivation events and educational meetings, public relations programs, and special events.
  • Act as liaison to family, community, and social service agencies.
  • Participate in civic clubs, local committees, chambers, etc.; serve on local boards when opportunities present.
  • Develop expertise of all comparable communities in the surrounding area.
  • Develop advertising, communications, and public relations strategies.
  • Maintain social media venues including Facebook, Internet web page, etc.
  • Assure compliance with regulatory requirements related to disclosure and marketing materials.
  • Handle concerns and/or problems that may arise in the office daily.
  • May not disclose any elder or facility information, any system log-in codes or password assigned or permit any unauthorized access to elder/resident or facility information without express written consent of the facility management.
  • Coordinate appointments for elders for referrals of sensory deficits including but exclusive to eye care, hearing care, dental care, psychiatric care, etc.
  • Provide information to elder and family members regarding available services of Long-Term Care State Ombudsman program.
  • Attend all required facility in-services and education.
  • Assist with coordination of a Resident Council and Family Council and render assistance to the Councils at the direction of and with the approval of the Councils.
  • Ensure that all concerns voiced during Council meetings are appropriately addressed and followed up on by the appropriate department representative.
  • Foster family and community support of the program through newsletters, networking, programs that bring family members or members of the community into the home, etc.
  • Serve as a member of Quality Assurance Performance Improvement Committee and Performance Improvement Plan meetings.
  • Serve as a member of the Behavior Management, Risk and Weight Loss Committees.
  • Maintain a clean, well-kept, professional appearance as it relates to office and presentation spaces.
  • Promote Wheat State Manor in a professional manner evidenced by appropriate hygiene and clothing.
  • Willingness to work beyond normal working hours and in other positions temporarily, when necessary.
