About The Position

Responsible for the provision of social work, spiritual care, music therapy (complementary care), and bereavement services to patients and families on a 24/7 basis. Serves as a psychosocial resource on the hospice management team in addressing high-risk and high-acuity patient and family needs, including but not limited to abuse and neglect, suicidality, medication diversion, issues of consent and autonomy, and discharge for cause. Manages the Hospice Social Work, Spiritual Care, Music Therapy, and Center for Grief Services departments, including but not limited to facilitating monthly departmental meetings, overseeing departmental budgets, and leading program development. Ensures all departmental services are delivered in compliance with applicable policies, regulatory standards, and organizational guidelines. Oversees the delivery of high-quality patient care within established departmental budgetary guidelines. Oversees the scheduling, coordination, and delivery of Counselor On Call services. Identifies hospice operational and clinical issues; collaborates with clinical managers on performance improvement initiatives to develop and implement solutions, evaluates outcomes, and uses data to drive ongoing improvements in service delivery. Responsible for the hiring, onboarding, and ongoing management of staff across all assigned departments, including payroll oversight, performance management, and staff recognition. Collaborates with clinical educators to support staff education and professional development, including teaching sections of monthly Pathways content. Collaborates with the Boccolini Institute in supporting internship opportunities in supervised departments. Provides ongoing coaching and counseling to staff on clinical and professional practice issues to support optimal performance. Collaborates with clinical management, the Vice President of Clinical Services, and Human Resources on departmental and organizational initiatives. Manages staffing resources in alignment with patient census and care needs. Participates in the manager on-call rotation with clinical managers to ensure 24-hour oversight of hospice services. Reviews patient documentation to ensure quality, timeliness, and regulatory compliance. Assists in the development and revision of policies and procedures. Serves on clinical and administrative committees as assigned and provides backup to staff Performs other duties as assigned. Participate in agency committees and represents Samaritan in community outreach endeavors

Requirements

  • MSW required
  • Valid NJ LSW, LCSW license preferred, valid NJ driver license and auto insurance
  • 5 years management experience required
  • Positive attitude, customer service skills, ability to manage and encourage change, understanding of/and commitment to hospice philosophy, ability to coach and mentor staff to promote professional development
