Provides the support and assistance to people we support to achieve skills necessary to live independently within the community and to create conditions within the homes to facilitate growth and self-awareness. The Manager of Community Home Services is responsible for supervision and oversight of the Team Leaders and Direct Support Professionals in the assigned Community Homes. The Manager plans, organizes, develops and directs the overall delivery of services in accordance with current federal, state, and program standards, guidelines and regulations as well as organizational policy and procedure. The Manager also recommends changes that would improve services performed. PERSON-CENTERED PRACTICES 1. Assures people have what is important to them present in their everyday life. 2.
