About The Position
Manages, supervises and assists the Director, Population Health Management (PHM) with oversight of short-term case management (CM) therapy services to facilitate high quality, cost effective care for Community First Health Plans, Inc. (Community First) membership. Works with staff at all levels of the health plan to facilitate member satisfaction and efficiency.
Requirements
- Master's Degree in Speech Language Pathology.
- Minimum of three years of clinical experience with two years of management experience is required.
- Referral management experience is required.
- Working knowledge of UM criteria ( and health care coding (ICD-9, CPT, HCPCS, DRG) is required.
- A current license to practice professionally as a Speech Language Pathologist (SLP) issued by the Texas Department of Licensing and Regulation for Speech-Language Pathology and Audiology is required.
Nice To Haves
- HMO experience is preferred.
