Position Summary The Manager, Population Health is a key contributor to the success of the organization’s clinical model, with a focus on improving health outcomes and managing medical costs. This role leads strategic and operational initiatives, including care program development, clinic support, and stakeholder engagement. Responsibilities span program implementation, reporting and analytics, and evidence-based care design. Ideal candidates bring 7–10 years of experience, including direct patient care, and demonstrate strong problem-solving, communication, and analytical skills. The position reports to the Director of Population Health Strategy & Analytics and offers significant growth potential.

Requirements

  • Experience: 7-10 years of prior relevant experience with a minimum of 2 years of experience with direct patient care
  • Strong MS Excel skills

Responsibilities

  • Key focus on health outcomes and controlling medical costs in our patient population
  • Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)
  • Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelines
  • Reporting and analytics in support of our Care Teams (physicians, nurses, care managers)
  • Establishing and maintaining relationships with both internal and external stakeholders
  • Other duties, as assigned
