POSITION SUMMARY The Community Habilitation Manager is responsible for overseeing a caseload of individuals and staff. The manager is responsible for training staff to assist individuals to become more independent when performing activities of daily living and community integration in order to achieve their valued outcomes. Services are primarily community based and revolve around volunteer and recreational opportunities. The Community Habilitation Manager is responsible for ensuring that the Community Direct Support Professionals (CDSPs) foster and maintain an environment that promotes the health and safety of all persons served. This exempt position is expected to maintain a regular, full-time work schedule of five (5) days per week and eight (8) hours per day, unless an alternative arrangement is documented and approved by leadership.

Requirements

  • High School Diploma or equivalent required
  • Minimum of two (2) years of experience supporting individuals with developmental and intellectual disabilities and/or mental health disorders required.
  • Valid CPR/FA certification required (provided upon hire as needed)
  • Fundamental computer skills with the ability to learn and use various applications
  • Proficient writing skills with grasp of basic grammar and spelling
  • Effective decision-making and time management skills
  • Knowledge of applicable OPWDD regulations
  • Ability to problem-solve and address conflicts and concerns within assigned program(s)
  • Must be efficient, well organized, and have excellent communication and interpersonal skills
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
  • 18 years of age or older
  • Valid Driver’s License and satisfy criteria to be a n agency driver required
  • Ability to work a standard, full-time schedule of five (5) days per week and eight (8) hours per day

Nice To Haves

  • One (1) year of supervisory experience is preferred.
  • Internal and external candidates with less than one year of supervisory experience will need to successfully complete internal management and supervisory skills training within 120 days of hire or transfer into the role. For any candidate with one or more years of supervisory experience, this is highly preferred.

Responsibilities

  • Provides oversight of the individual’s direct services that support them in a least restrictive manner.
  • Creates habilitation goals and trains CDSPs to support participants in identifying meaningful activities aligned with their Life Plan, promoting independence and achievement of personal outcomes.
  • Coordinates and delivers habilitation services, including training, supervision, and support in areas such as communication, interpersonal skills, socialization, mobility, community travel, problem-solving, money management, and life safety.
  • Supervises and supports assigned staff, including administrative responsibilities such as payroll, training compliance, and performance management.
  • Interviews, hires, develops, and retains high quality staff
  • Ensures program roster(s) are accurate
  • Oversees staff documentation to ensure compliance with regulations, policies, and procedures; reviews and revises submissions as needed.
  • Responds to crisis and emergency situations to ensure participant safety and well-being, following agency protocols, proper reporting procedures, and collaborating with support networks to identify risks and implement safeguards.
  • Addresses challenging behaviors with the participants and CDSPs as needed.
  • Attends and completes habilitation plan meetings.
  • Attends IDT/team/ISP-Individualized Service plan meetings for assigned caseload as well as any required behavioral trainings for the individual or Community Habilitation program.
  • Maintains ongoing contact with the individual’s family, case manager, management team, etc.
  • Creates and maintains all individuals records/update accordingly and trains the CDPs on the individual’s background information.
  • Completes screenings/enrollment of individuals, required documentation for enrollment, and meet and greets.
  • Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
  • Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
  • Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.
  • Ensures compliance with FREE policies, procedures, and operating standards.
  • Implements a continuous quality improvement process for department services and administrative functions.
  • Assists in the development, implementation and administration of Human Resources policies and programs including compliance with Equal Employment Opportunities and Affirmative Action Programs.
