Requirements

  • Current RN, PT, OT, or Speech Language Pathologist license in the state of Minnesota.
  • Current CPR certification.
  • Strong leadership, team management, and clinical oversight skills.
  • Knowledge of Medicare, Medicaid, and home health regulations.
  • Strong communication, collaboration, and problem-solving skills.
  • Valid driver's license and access to an insured personal vehicle.
  • Ability to pass state mandated background check.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with staff, clients, and referral sources.

Nice To Haves

  • Bachelor's degree preferred.
  • Minimum 1 year of home health experience preferred.

Responsibilities

  • Lead Clinical Operations - Oversee the interdisciplinary team to ensure timely, appropriate, and high-quality care while maintaining regulatory and billing compliance.
  • Supervise and Support Staff - Manage field and office staff to optimize care delivery, meet productivity standards, and maintain best-practice team utilization.
  • Ensure Efficient Case Management - Assign caseloads based on client acuity, staff availability, and performance benchmarks to balance workload and quality outcomes.
  • Drive Quality and Compliance - Partner with Quality and Compliance teams to analyze clinical outcomes, CAHPS scores, Home Health Compare data, and PEPPER reports, implementing action plans for improvement.
  • Foster Growth and Retention - Collaborate with HR, Training, and Quality teams to provide strong onboarding, ongoing education, and staff development for optimal team retention and clinical excellence.
