Requirements

Current RN, PT, OT, or Speech Language Pathologist license in the state of Minnesota.
Current CPR certification.
Strong leadership, team management, and clinical oversight skills.
Knowledge of Medicare, Medicaid, and home health regulations.
Strong communication, collaboration, and problem-solving skills.
Valid driver's license and access to an insured personal vehicle.
Ability to pass state mandated background check.
Physical capability to perform all essential job functions.
Ability to read, write, and speak English to ensure effective communication with staff, clients, and referral sources.

Nice To Haves

Bachelor's degree preferred.
Minimum 1 year of home health experience preferred.

Responsibilities

Lead Clinical Operations - Oversee the interdisciplinary team to ensure timely, appropriate, and high-quality care while maintaining regulatory and billing compliance.
Supervise and Support Staff - Manage field and office staff to optimize care delivery, meet productivity standards, and maintain best-practice team utilization.
Ensure Efficient Case Management - Assign caseloads based on client acuity, staff availability, and performance benchmarks to balance workload and quality outcomes.
Drive Quality and Compliance - Partner with Quality and Compliance teams to analyze clinical outcomes, CAHPS scores, Home Health Compare data, and PEPPER reports, implementing action plans for improvement.
Foster Growth and Retention - Collaborate with HR, Training, and Quality teams to provide strong onboarding, ongoing education, and staff development for optimal team retention and clinical excellence.
