About The Position

We are currently looking for a Manager of Community Partnerships to develop and Maintain relationships between Hospice of the Chesapeake (HOC) and various referral sources in their assigned territories. The focus of these relationships is to improve patient access to HOC services, increase overall census and educate referral sources about the benefits to patients and families. Requirements

  • Bachelor’s Degree or equivalent combination of education and experience, in marketing, nursing, healthcare administration, social work, business administration or related field, required; a successful track record in outside sales/marketing highly desirable.
  • Minimum of three (3) years successful sales/marketing experience in healthcare. Experience in hospice, home health, or durable medical equipment (DME) highly desirable.
  • Demonstrated knowledge and understanding of managed care organizations, including utilization review and capitated contracting concepts with hospitals and physician groups (physician billing, nursing home billing and hospice Medicare billing.
  • Solid analytical, organizational and interpersonal skills in order to effectively identify referral source problem areas, develop appropriate courses of action and convey same to client.
  • Must be computer fluent and able to use email, and common business software (i.e. Microsoft Word, Excel and PowerPoint) to conduct business in a timely and accurate manner.
  • Current Maryland driver’s license and clean driving record. Reliable transportation a must.

Responsibilities

  • Cultivates trusting relationships with healthcare professionals in assigned accounts and service areas to maintain consistent awareness of HOC's programs and services.
  • Analyzes referral source patterns and partner needs. Develops personalized outreach strategies that strengthen existing partnerships and nurture new relationships to extend our circle of care.
  • Partners with the Communications department in the development of educational, marketing, and communication pieces that effectively translate the clinical approaches and “image” of HOC.
  • Conducts liaison and admissions visits as necessary and appropriate to licensure. Coordinates with Access Services department to facilitate these visits.
  • Serves as a compassionate ambassador for HOC at professional/ clinical events. Prepares and makes presentations.
  • Leads or participates in a variety of Business Development, Clinical Teams and other meetings and activities.
  • Other duties as assigned.
