About The Position

The Manager of Case Management (MCM) is responsible for directing the processes and staff for a team of individuals working clients utilizing shelter or residential programs with the goal of returning individuals to stable housing solutions.

Requirements

  • Bachelor’s degree in social work-related field plus one year of related work experience OR three years of related work experience required. Masters in social work-related field preferred.
  • A combination of education and experience may be substituted.
  • Colorado Certified Addiction Specialist (CAS) certification or equivalent preferred.
  • Experience managing employees is preferred.
  • Schedule will include early mornings or late evenings or both.
  • Active Driver’s License and insurable under Denver Rescue Mission’s standards to travel to various DRM facilities, partner organizations, and housing properties.
  • Ability to obtain and maintain CPR certification within 90 days of hire.
  • Must be a Christian holding to the beliefs set forth in the Statement of Faith, expressed by a personal testimony and Christian conduct.
  • Excellent verbal and written communication skills, and responsiveness to communication.
  • Ability to resolve conflict professionally and respectfully while maintaining composure under pressure.
  • Proficiency with Microsoft Office 365 applications, especially Excel.
  • Preferred experience with Homelessness Management Information System (HMIS).
  • Knowledge in Behavioral Health approaches, including Trauma Informed Care, Motivational Interviewing, Crisis Intervention and De-escalation strategies.
  • May be necessary to drive clients to appointments, must be willing to perform this task.

Responsibilities

  • Foster a culture of grace amongst staff that orients them around the mission of meeting individuals at their physical and spiritual points of need.
  • Manage the staff experience: Recruiting, interviewing, hiring, onboarding, training, resolving conflict, payroll, scheduling time off and quarterly reviews for staff.
  • Provide regular supervision of staff by convening weekly team and individual meetings.
  • Provide continual oversight and training to case management staff ensuring that they remain housing-focused, and client centered evaluating engagement and problem solving with staff.
  • Ensure that all case managers are entering the appropriate data into Homelessness Management Information System (HMIS) and SCRIBE regarding the individualized housing plans and assessments to facilitate outcome reports to both internal and external partners.
  • Attend internal DRM Meetings that promote collaboration.
  • Attend weekly meetings with key staff members that promote collaboration and good relations with all DRM Departments.
  • Represent the Mission at meetings and on committees as directed.
  • Communicate with the public through speaking arrangements, community meetings, interviews, etc.
  • Work closely with Managers on other DRM teams to ensure Continuity of Care within the internal DRM system.
  • Ensures data collection and reporting for outcomes is completed for external contract(s) and internal DRM requirements and reporting.
  • Conducts 1:1 meetings with Case Management staff weekly or biweekly as directed.
  • Maintain the relationships necessary to ensure DRM’s case management efforts reflect a real-time understanding of broader re-housing efforts, availability, requirements, and protocol.
  • Conduct case audits to ensure quality and consistency of staff documentation and case load engagement.
  • Ongoing audit of grant performance and development of processes and workflows.
  • With the help of DHROT- Oversee budgets and work with Accounting Team in obtaining needed funds for Client Housing placements and other client services engaged.
  • Discuss with DHROT the development of processes and prompts within SCRIBE to facilitate proper recording and coordinate efforts toward HMIS training and reporting.
  • Ensures all team paperwork is current with best practices and implements changes pending Director approval.
  • Lead internal DRM Case Management meeting to discuss internal case conferencing solutions.
  • Other duties as assigned.
