Purpose of Position: Oversee and maintain the operations of the CAMP (Chronic Care Active Management and Prevention) Department. Direct assigned employees, providing leadership and supervision for CAMP Department Programs. Ensure the development of culturally sensitive programs and education that promotes physical and emotional wellness to prevent chronic illnesses and improve quality of life. Uphold the organization s vision, mission, and corporate values. Demonstrate understanding of and compliance with organization s policies, procedures, code of conduct and work rules. Uphold the CAMP Department s Mission to promote smart choices, strong bodies and sustainable lifestyles. Essential Functions: Perform leadership functions including selection, scheduling, training, timecard approvals, performance management and discipline. Instill passion in assigned staff for NSHC s mission, vision, and values, paying particular attention to the maintenance of a safe and productive work environment. Direct assigned employees, demonstrating leadership by providing work direction, motivation, training, assistance, performance evaluation and corrective action. Supervise the day-to-day operations of CAMP programs. Train staff on the processes and protocols of the program s activities. Support programing to include but not limited to outpatient referral based services, health promotion, medical nutrition therapy, diabetes education, biometric health screenings, weight management and support, physical fitness management and classes, health fairs, summercise, community health education, walks/runs events, tobacco cessation counseling, tobacco prevention education and outreach, injury prevention education and outreach, injury prevention store, lactation counseling, childbirth education and classes and food literacy and access initiatives. Assist with the management of the budget and securing grants to further the program mission. Measure outcomes and compile results. Assist with grant management, monitoring and reporting to satisfy grant requirements. Demonstrate personal initiative and accountability while overseeing the department's budget; Forecast needs, allocate resources and/or take corrective action as necessary. Support referrals in outpatient, inpatient, and community settings. Serve as the liaison between CAMP and medical staff, facilitating collaboration when needed. Document services appropriately in the electronic health record. Collaborate with communities of the region and assuring availability of outcome data, coalition and team building. Provide educational opportunities and events. Work with communities and staff to set and achieve community-based goals related to wellness, nutrition, diabetes education, etc. Document achievements. Compile and evaluate data and provide reports to the Wellness Director, NSHC Administration and Board of Directors; report to other agencies including but not limited to IHS Special Diabetes Program for Indians and the State of Alaska, etc. Develop and maintain memorandums of agreement and solid working relationships with an array of community-based organizations in Nome and surrounding villages and represent NSHC effectively in meetings with State, I.H.S and other agency officials.
