About The Position

The Manager of Case Management is responsible for oversight and management, operations and ongoing quality, productivity and efficiency for case management processes. Coordinates with facility and service line leaders to ensure continuity of care throughout the continuum. Connects with community resources and leaders to establish and maintain relationships. The Manager of Case Management for Community Alternatives Program (CAP) is responsible for oversight and management, operations and ongoing quality, productivity, and efficiency for CAP and acute care case management processes. Coordinates with CAP Service providers, facility and service line leaders to ensure continuity of care throughout the continuum. Connects with community resources and leaders to establish and maintain relationships.

Requirements

  • Education: 4 Year / Bachelors Degree,required. Bachelor's Degree in Nursing or Bachelor's Degree in Social Work accredited by the Council on Social Work Education or Bachelor's Degree in related healthcare field.
  • Experience: Three years experience in Case Management or related experience, required.
  • Licensure/Certification: If RN, current RN licensure in appropriate state, required.
  • Additional Skills (required): Current and future trends/practice in area of responsibility; regulatory and governing standards, policies and procedures; information systems and software used in area of responsibility; equipment used in performing assigned duties; application of safety and infection control policies and procedures; skills in interviewing and selection process; understanding of budgeting, staffing, payroll and purchasing processes; implementation of competency, feedback, development and progressive discipline processes; teambuilding; conflict management, and interpersonal relationship skills; principles of adult learning and teaching methods/models; decision making, project leadership and consultation; delegation and communication skills; understanding of customer needs and expectations and knowledge for improvement to exceed customer expectations.

Nice To Haves

  • Graduate Degree, preferred.
  • Leadership experience, preferred.
  • Certification in Case Mgmt. or related field, preferred.
  • Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
