About The Position

The Manager of Case Management is responsible for overseeing the performance of all Case Management (CM) staff duties. This includes assisting with the management of discharge planning from the beginning of the admission through to the transition to the next level of care. The manager will also oversee the performance of all CM campus staff activities and work schedules, respond to daily operations of the departments, and maintain a staffing plan that meets organizational needs. Additionally, the manager will maintain a system for reporting results, be responsible for CM staff development, and work with hiring, firing, orientation, training, and evaluation according to established guidelines. The role includes managing quality control and performance improvement activities, ensuring compliance with productivity and budgetary standards, and adhering to all Regulatory Agencies. The manager will assess, develop, and evaluate departmental goals on a yearly basis, communicate departmental needs, and recommend capital equipment purchases. Collaboration with Corporate and System Leaders as well as Administration in developing and implementing standards of work for the STHS facilities is also a key responsibility.

Requirements

  • Minimum of 3 years experience in Case Management or related medical field.
  • A current Texas RN License or BSW/LMSW is required.
  • Current CPR certification.
  • Knowledge of performance standards as outlined by Centers for Medicare & Medicaid Services.
  • Membership in related professional organization preferred.
  • Strong interpersonal skills and experience in team development.
