We manage services for individuals with serious mental health needs, substance use disorders, traumatic brain injuries, and intellectual/development (IDD) disabilities. Trillium Health Resources has a career opening for a Care Coordination Manager to join our team! The Care Coordination Manager oversees a skilled, multi-disciplinary team of professionals who provide localized support within a managed care environment to members living with behavioral health, mental health, substance use, and intellectual/developmental disabilities or traumatic brain injury. The Care Coordination Manager ensures staff implement industry standards, evidenced-based and best practices to support members. The Care Coordination Manager provides oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better.

Requirements

  • Master’s degree in a Human Services field and three (3) years’ experience working with behavioral health, mental health, substance use, and/or intellectual/developmental disability populations. OR Equivalent combination of education/experience.
  • Fully licensed in the State of North Carolina as an LCSW, LCMHC, LMFT, or LPA.
  • Must have a valid driver’s license
  • Must reside within North Carolina
  • Must be able to travel within catchment as required.

Nice To Haves

  • Work experience focused on working with individuals with behavioral health, mental health, substance use, and/or intellectual/developmental disability populations.
  • Supervision of two (2) or more employees.

Responsibilities

  • Provide direct supervision and clinical oversight for a localized, multi-disciplinary team supporting members living with mental health, substance use, and intellectual/developmental disabilities.
  • Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently.
  • Monitor teams to help ensure integration of care through the establishment of a multi-disciplinary team (e.g. member, caretaker/legal guardian, PCP, behavioral health provider, specialists, nutritionists, pharmacy, etc.) to meet the member’s needs.
  • Be familiar with evidenced-based and best practices for members and translate that knowledge into specific processes, procedures, and workflows for staff to identify and facilitate effective and efficient coordination   of care for members.
  • Use knowledge and skills to create and identify methodologies and tools to be used in the execution of care coordination.
  • Coach team members to use member engagement practices that support the engagement of members throughout the continuum of care.
