About The Position
Oak Glen Post Acute is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our skilled nursing team. The LVN will provide high-quality nursing care to residents while working collaboratively with RNs, physicians, and interdisciplinary staff to support resident health, safety, and comfort.
Requirements
- Current Licensed Vocational Nurse (LVN) license in the state of California
- CPR certification (BLS)
- Experience in skilled nursing or long-term care preferred
- Strong clinical, communication, and documentation skills
- Ability to work effectively in a fast-paced healthcare environment
- Commitment to providing compassionate, resident-centered care
Nice To Haves
- Experience in skilled nursing or long-term care preferred
Responsibilities
- Provide direct nursing care to residents in accordance with facility policies and procedures
- Administer medications and treatments as ordered by physicians
- Monitor residents’ conditions and report changes to the RN or physician
- Accurately document care, treatments, and resident responses in medical records
- Assist with admissions, discharges, and care planning
- Ensure compliance with infection control, safety, and regulatory standards
- Support residents and families with professionalism, empathy, and respect
- Collaborate with the interdisciplinary care team to deliver person-centered care
