About The Position

Sunnyvale Gardens Post Acute is currently seeking a Full-Time Licensed Vocational Nurse (LVN) for our NOC (overnight) shift. This is an excellent opportunity to join a dedicated clinical team in a supportive post-acute care environment. Requirements

• Current California LVN license in good standing
• Strong clinical and communication skills
• Ability to work independently and as part of a team
• Reliable and committed to high-quality resident care

Nice To Haves

• Long-term care or post-acute experience preferred

Responsibilities

• Provide direct patient care in accordance with facility policies and standards
• Administer medications and treatments as ordered
• Monitor patient conditions and report changes
• Accurately document care and services provided
• Collaborate with RNs, CNAs, and interdisciplinary team members
• Ensure compliance with safety, infection control, and regulatory guidelines
