About The Position

Great opportunity to join an award-winning team! You would be responsible for the nursing needs of residents, while actively incorporating our “Philosophy for Person-Directed Care.” in a skilled nursing setting. Ensures a safe and healthy environment for residents, staff, and visitors to participate in the resident’s daily life rhythm. Develops assignments in coordination with the care partners in the health center and keep each team member accountable for resident care and satisfaction. Lead and direct a team of caregivers in an assigned area. Duties and assignments may be adjusted at the discretion of the Director of Nursing Services and/or designee. Pay range: $37.00 - $56.00 an hour depending on experience. This is an on-call position for AM, PM and NOC Shift. Days may vary.

Requirements

  • Current LVN/LPN in the state for which applying
  • IP Certified or willing to obtain within 30 days
  • Current CPR certification
  • Prior nursing experience: senior care, skilled nurse, post-acute or sub-acute care preferred

Responsibilities

  • Takes responsibility to lead the team and ensures that all procedures are followed during shift
  • Provides resident care, including medication set up
  • Maintain communication with doctors, laboratory, pharmacy and residents’ families concerning resident care
  • May supervise and direct care of residents in assisted living and those in Residential Plus
  • Responds to emergency calls in residential living and assisted living
  • Notes safety hazards in residents’ apartments and monitors and provides safety instruction for residents
  • Provides input for the Resident Review Committee and may participate in the Service Plan in order to evaluate health status of residents who have been having difficulties; confers on measures for assisting or intervening
  • Serves as a nurse in Wellness Clinic
  • Conducts initial annual resident assessments as well as assessments for change of condition
  • Performs health updates, interviews, and charting; maintains daily records, takes doctors’ orders
  • Promotes and protects the rights of each resident
  • Performs other related essential duties as required
