About The Position

We are seeking a compassionate and skilled LVN/LPN to provide quality nursing care in a long-term care and rehabilitation setting. The role involves supporting a diverse patient population, including individuals with Alzheimer’s and Dementia, while ensuring high standards of patient care and safety.

Requirements

• LPN Diploma
• Active New York (NY) LVN/LPN License (required at submission – no restrictions)
• BLS Certification required
• Minimum 1+ year of LTC experience (flexible)
• First-time travelers accepted
• Local candidates accepted
• COVID-19 vaccination required
• Must be available to work every other weekend

Nice To Haves

• Experience with PointClickCare EMR (not required)

Responsibilities

• Perform medication administration (med pass) and prescribed treatments
• Monitor and record patient vital signs and overall condition
• Provide care to residents in a Rehab and nursing facility
• Support patients with Alzheimer’s and Dementia
• Maintain accurate and timely patient documentation
• Collaborate with interdisciplinary healthcare teams
• Educate patients and families on care plans and treatments
• Follow infection control and safety protocols
