Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. It is expected that the employee demonstrate behavior consistent with our core values\: Integrity, Accountability, Best Practices, Compassion and Synergy. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. The Hospice LVN provides intermittent visits to the terminally ill patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the end of life

Requirements

  • Minimum 2 year’s relevant experience as an LVN in an acute care setting.
  • Current LVN license issued by the California Board of Registered Nursing
  • BLS issued by the American Heart Association

Nice To Haves

  • Home Health, Oncology, and/or Hospice experience preferred
  • Electronic Medical Record experience

Responsibilities

  • Assesses present and potential needs of patient and family
  • Review Plan of Care with patient, family, physician, IDT and review and evaluate
  • Review realistic goals of care to ensure continuity and quality of care
  • Complete all documentation in a concise, timely manner per agency standards
  • Manage symptoms and teach patient/family care management
  • Ability to be at work and on time
  • Ability to follow company policies, procedures and directives.
  • Ability to interact in a positive and constructive manner.
  • Ability to prioritize and multitask.
