About The Position

Join Our Team at Lutheran Village at Miller's Grant! We’re currently seeking a dedicated Full-Time LPN Memory Care Coordinator to become part of our caring and mission-driven community. This position will be Monday-Friday, 9:00am-5:00pm, with every other weekend rotation. The Memory Care Coordinator is responsible for overseeing the daily operations of the Assisted Living Dementia Care Unit, ensuring compliance with all applicable federal, state, and local regulations. This role focuses on delivering the highest standard of care, promoting a safe and supportive environment, and enhancing the quality of life for residents living with dementia. Requirements

  • Associate’s degree in a related field, with advanced training or demonstrated knowledge in dementia care.
  • 2-3+ years of relevant experience, or an equivalent combination of education and experience.
  • Current certifications and licensure as required by applicable state regulations.
  • Demonstrated discretion, professionalism, and sound judgment in handling sensitive and confidential matters.
  • Excellent interpersonal, written, and verbal communication skills, with the ability to interact effectively and tactfully with individuals at all levels, including residents, staff, families, external vendors/authorities, and the public.
  • Strong attention to detail in writing, proofreading, managing priorities, and meeting deadlines in a fast-paced environment.
  • Proven ability to work both independently and collaboratively within a team environment, demonstrating a cooperative, respectful, and compassionate approach to colleagues and resident care.
  • Skilled in the use of standard office equipment and technology.
  • Maintains professionalism, composure, and a positive attitude when managing challenging situations.
  • Physically able to move throughout the workday and lift items up to 30 pounds using safe handling practices.
  • May be necessary to assist with resident evacuation during emergency situations.
  • Complies with organizational health and safety requirements, including vaccination policies or valid exemptions.

  • Licensed Practical Nurse preferred.

Responsibilities

  • Ensure daily dementia programming is planned, implemented, and aligned with resident abilities and interests.
  • Serve as the primary point of contact for residents and their families, fostering open communication and trust.
  • Coordinate and provide ongoing dementia-specific training and education for care team members.
  • Ensure timely implementation and ongoing review of resident service plans.
  • Collaborate with care staff and the Delegating Nurse to ensure residents receive appropriate assistance with activities of daily living (ADLs), medication management, and healthcare needs.
  • Recruit, hire, train, and supervise care team staff in partnership with the Assisted Living Coordinator.
  • Coordinate resident move-ins in collaboration with families and the Delegating Nurse.
  • Support the Delegating Nurse in assessments and the development of person-directed service plans for new and existing residents.
  • Conduct daily rounds to assess residents’ cognitive and emotional status, care effectiveness, and team engagement.
  • Inspect the environment and equipment daily for cleanliness, safety, and maintenance needs; report concerns using WorxHub.
  • Foster a positive, engaging community atmosphere that encourages social interaction and a sense of belonging.
  • Participate in quality assurance initiatives related to memory care and overall assisted living standards.
  • Coordinate staffing schedules in collaboration with the Health Care Center Scheduler to ensure adequate coverage.
  • Attend and contribute to meetings, in-services, educational sessions, and designated committees.
  • Maintain up-to-date knowledge of dementia care best practices, regulations, and company policies.
  • Ensure consistent attendance and active participation in scheduled work and meetings.
  • Perform additional duties as assigned.
