POSITION SUMMARY PLEASE NOTE: This position will be on-site. We are seeking an intake worker for behavioral health and substance abuse intervention. The intake worker would be part of the behavioral health team, working in collaboration with providers and community health workers.

Requirements

  • Bachelor’s degree in social work, psychology, or a related field
  • Experience working in a similar role or within social services
  • Strong knowledge of community resources and services
  • Excellent communication and interpersonal skills
  • Experience with case management software
  • Strong organizational and time-management skills
  • Ability to handle sensitive information with discretion
  • Proficient in Microsoft Office Suite
  • Excellent problem-solving abilities
  • Empathetic and non-judgmental attitude
  • Ability to work independently and as part of a team
  • Strong written and verbal communication skills
  • Experience in customer service or client support

Responsibilities

  • Conduct initial client assessments to gather detailed information
  • Evaluate clients' needs and determine eligibility for services
  • Identify appropriate levels of care and refer accordingly
  • Maintain accurate and up-to-date client records
  • Coordinate with other service providers and agencies
  • Follow up with clients to monitor progress and address any additional needs
  • Assist clients in completing necessary paperwork and applications
  • Ensure confidentiality and privacy of client information
  • Participate in team meetings and case conferences
  • Stay informed about community resources and services
  • Provide crisis intervention and support as needed
  • Advocate on behalf of clients to access necessary services
  • Develop and maintain positive relationships with clients and service providers
