Requirements

A current LPN license in New York State.
The ability to remain calm and effective under pressure.
Strong communication and interpersonal skills.
The ability to work effectively in a team environment.
Time management skills.
A passion for caring for residents in a respectful, caring, and professional manner

Nice To Haves

Previous experience in long term care preferred.

Responsibilities

Provide direct patient care under the supervision of an RN.
Monitor resident vitals, administer medications, and assist with treatments.
Assist with daily living activities and resident care as needed.
Work collaboratively with nursing leadership and community interdisciplinary team to communicate resident progress and concerns.
Document resident care accurately and maintain electronic medical records.
Maintain a safe, clean, and organized environment for colleagues, residents, and visitors.
Act as a liaison between the nursing team, providers, and residents and their family members.
Ensure compliance with regulatory, safety, and infection control standards and protocols.
