Requirements

  • A current LPN license in New York State.
  • The ability to remain calm and effective under pressure.
  • Strong communication and interpersonal skills.
  • The ability to work effectively in a team environment.
  • Time management skills.
  • A passion for caring for residents in a respectful, caring, and professional manner

Nice To Haves

  • Previous experience in long term care preferred.

Responsibilities

  • Provide direct patient care under the supervision of an RN.
  • Monitor resident vitals, administer medications, and assist with treatments.
  • Assist with daily living activities and resident care as needed.
  • Work collaboratively with nursing leadership and community interdisciplinary team to communicate resident progress and concerns.
  • Document resident care accurately and maintain electronic medical records.
  • Maintain a safe, clean, and organized environment for colleagues, residents, and visitors.
  • Act as a liaison between the nursing team, providers, and residents and their family members.
  • Ensure compliance with regulatory, safety, and infection control standards and protocols.
