Requirements

• Valid LPN license in PA.
• Ability to work effectively in memory care units and long-term care facilities.
• Strong understanding of acute pain management techniques.
• Contingent upon background checks.

Responsibilities

• Provide high-quality nursing care to patients in a long term care and short term rehab setting.
• Administer medications, perform wound care, monitor patient vital signs, and assist with treatments as directed by healthcare providers.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents.
• Educate residents and their families on health management strategies and treatment plans.
• Ensure accurate documentation of patient information and treatment procedures in electronic health records.
