Requirements

• Education: High School Diploma or equivalent required.
• Licensure: Valid LPN license in Georgia required.
• Experience: Minimum of 3 years of medical/surgical nursing experience in an acute care setting in a hospital environment required.
• Certifications: Basic Life Support (BLS) certification from AHA required.
• Skills & Abilities: Strong clinical assessment skills and attention to detail.
• Excellent organizational, time-management, and problem-solving abilities.
• Exceptional written and verbal communication skills.

Responsibilities

• Patient Care & Coordination: Provide direct patient care, including assessments and medical procedures, under RN supervision.
• Follow physician orders and the nursing process to manage patients’ total needs.
• Coordinate patient care with the RN, therapists, and other team members, ensuring continuity of care.
• Report changes in patients’ conditions and contribute to care plan adjustments.
• Meet established productivity standards and deliver patient-centered care.
• Patient & Caregiver Education: Educate patients and caregivers on self-care, promoting independence and safety.
• Actively monitor and report any safety concerns to appropriate authorities or leadership.
• Communication & Documentation: Complete accurate and timely documentation according to regulatory guidelines.
• Actively participate in care team meetings and contribute to collaborative problem-solving.
• Staff Education & Professional Growth: Stay current with clinical knowledge by attending continuing education, in-services, and staff development activities.
• Additional Responsibilities: Participate in special projects and assist with other duties as assigned by leadership.
• Comply with all Wellstar Health System policies, procedures, and standards of care.
