About The Position

Requirements

• Current and valid NY State LPN license.
• Minimum one year of experience in a long-term care or similar healthcare setting.
• Strong communication, assessment, and organizational skills.
• Ability to work efficiently both independently and as part of a healthcare team.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and assess patients' health status, documenting changes appropriately.
• Assist residents with activities of daily living and ensure their comfort and safety.
• Collaborate with the interdisciplinary healthcare team to implement effective care plans.
• Maintain compliance with facility policies, state and federal regulations.
• Provide emotional support and education to patients and their families.
