Provides direct nursing care in accordance with established policies, procedures and protocols. Essential Functions: Ready the exam room with the appropriate equipment before patient visits. Set up all procedures and assist the provider during the patient visit as needed. Room patients, prepare them for the provider, and discharge them as per written protocol. Per provider order and/or standing orders, perform POC (Point of Care) tests. Perform venipuncture for blood collection. Administer and chart subcutaneous (SC), intramuscular (IM), or intradermal (ID) injections and/or vaccinations, and medications per provider order or standing orders. Maintain required education for and working knowledge of VFC (Vaccines for Children) or other vaccination programs clinic participates in. Set up patient oxygen and administer O2 to patient via appropriate delivery and perform 3-point testing for oxygen recertification. Keep assigned rooms cleaned after each patient and stocked appropriately and assist with maintenance of inventory as assigned. Manage prescription refill requests. Follow up on prescriptions, lab orders, and testing Pre-visit plan for upcoming scheduled days. • Provides Marillac patients with prescribed medications and DME supplies (not including oxygen). • Documents the patient’s complete list of active medications, which includes prescribed medications, over the counter medications and nutraceuticals (herbals/supplements). Creates refill requests from phone, voicemail, pharmacy, electronic medical record e-script, patient portal, in person- Patient is present in the clinic. • Starts and routes prescription requests to the appropriate provider, including all pertinent data. Verifies medication strength, frequency, and that the medication can be refilled or not. Complete Rx refills per protocol. Develops/updates/follows medication refill protocols. • When the request is for a new medication or a discontinued medication helps the patient make an appointment to be seen to discuss the need for this medicine with their provider. • Does pre-authorizations for medications, PDMP reports. • Keeps patient controlled substance agreements current. • Manages prescription inbox in the electronic medical record for out-of-town providers- route to the covering providers’ inbox. • Sends contact letters to patients if unable to reach by phone stating the need for an appointment before further refills. • Helps the call center during high volume times, or when an associate is out sick/vacation. • Verifies patient information is correct in the electronic medical record before sending refill-insurance, pharmacy, address, emergency contacts. • Makes appointments for patients as needed, routes incoming calls to the appropriate person/department. • Perform office phone medical triage for patients with medical questions or concerns to determine appropriate scheduling for patient in a tactful, confidential and professional manner. • Documentation of ALL activities completed will be charted within the patient’s medical record. • Participation in clinic quality measures is mandatory. • Perform other duties as assigned. Competencies: • Good Proficiency and navigation PC application included by not limited to MicroSoft suite of products and EMR systems • Ability to multitask while actively listening • Excellent keyboarding skills • Good telephone manner with a clear speaking voice • Effective communication skills and the ability to build rapport with the patients, outside professionals and staff • Ability to work in a PCHM team model of care
