About The Position

Requirements

  • Current LPN (Licensed Practical Nurse) license in the State of Minnesota
  • Minimum of 1 year experience in assisted living, home health, or long-term care preferred
  • Strong organizational skills with the ability to work independently
  • Passion for working with seniors and providing compassionate care

Responsibilities

  • Assures residents maintain admission criteria; monitors and recommends alternative placement if necessary
  • Ensures residents have up-to-date care plans in compliance with state regulations
  • Monitors and maintains resident charts and clinical progress notes
  • Communicates changes in resident condition to physicians, staff, and families
  • Coordinates resident services with other providers
  • Oversees medications, renewals, and physician follow-up
  • Ensures proper medication administration procedures are followed and addresses any concerns or errors
  • Assists with medication administration as needed
  • Provides hands-on resident care intermittently to support the care team
  • Documents and communicates important updates in the resident log
  • Conducts supervisory visits in coordination with the RN
  • Works collaboratively with the full care team
  • Participates in a rotating on-call schedule to support clinical and staffing needs
  • Oversees medication practices and medical equipment use
  • Ensures staff follow all resident care procedures
  • Communicates resident and family concerns appropriately
  • Assists leadership in maintaining emergency preparedness procedures
