About The Position

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our care team. Under the direction of a Registered Nurse (RN), the LPN provides individualized, resident-centered nursing care to elderly, acutely ill, and injured residents. This role combines clinical expertise and compassionate care to support resident wellness, comfort, and recovery.

Requirements

  • Current, active Licensed Practical Nurse (LPN) license
  • Ability to respond quickly and effectively in emergency situations
  • Strong communication and interpersonal skills
  • Ability to work collaboratively in a team environment
  • Strong organizational and time management skills
  • Compassionate, positive, and resident-focused approach to care
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.

Nice To Haves

  • Six months experience in a long-term care environment preferred.

Responsibilities

  • Provide direct nursing care within scope of practice and competency
  • Collaborate with RNs using the SBAR process for assessments and recommendations
  • Participate in resident evaluations and care planning, including reviewing and updating care plans as appropriate
  • Coordinate care activities and communicate changes to interdisciplinary team members and families
  • Make rounds with physicians and care team members as needed
  • Supervise Medication Aides/Technicians in medication and treatment administration
  • Administer treatments and monitor resident responses
  • Administer medications safely and accurately, including post-admission doses
  • Prepare, administer, and document medications as prescribed
  • Monitor for and report medication effectiveness and adverse reactions
  • Maintain accurate narcotic records within scope of practice
  • Coordinate medication ordering and notify appropriate staff of unavailable medications
  • Complete medication-related documentation and reviews as assigned
  • Ensure medication/treatment carts are clean, organized, and stocked
  • Review, transcribe, and implement physician orders in compliance with state regulations
  • Communicate effectively with physicians and care team members
  • Assist in emergency situations and perform CPR as needed
  • Complete clinical admission paperwork and assist with discharge processes
  • Support consultants during visits and follow up on recommendations
  • Assist with medical record audits and implement pharmacy or drug regimen review recommendations
  • Accurately document care and resident status in the clinical record in a timely manner
  • Follow all company policies, procedures, and regulatory requirements
  • Ensure compliance with federal and state laws and professional standards
  • Maintain punctuality and consistent attendance for scheduled shifts
  • Perform other duties as assigned
