About The Position

As a member of our small nursing team, the LPN supports the coordination and direction of all our health care services including formal evaluations, education, programming, documentation and direct care duties. Expanded responsibilities and qualifications are as follows: · Provides direct medical care to residents as needed. · Completes medical referrals, reviews doctor’s orders and updates staff as needed. · Sets medications according to Restart, Inc. policy & procedures. · Helps ensure medication administration is safe and compliant. · Works with non-medical staff to ensure compliance and documentation is completed appropriately. · Acts as a resource to other staff regarding medical care for residents. · Updates and trains house staff, as needed, on Resident Care Plans and/or changes in care policy or procedure. · Participates in the on-call schedule. · Proactively contributes to a healthy and functional working and care environment.

Requirements

  • Current LPN license in good standing.
  • Ability to travel between sites as needed.
  • Ability utilize technology (EMR, Microsoft Office, etc.) relevant to the position
  • Ability to exercise independent decision-making skills within scope of license.
  • Excellent verbal and written communication skills.
  • Pass all required safety and background checks required by regulation and Restart policy

Nice To Haves

  • Previous experience working in a residential or assisted living environment highly preferred.
