About The Position

Lorien Health Services is hiring Licensed Practical Nurses (LPNs) at their Lorien Columbia community. This role offers opportunities for patient-centered care and career growth within an award-winning healthcare team. Lorien Columbia, the flagship location of Lorien Health Services, has been serving Howard County, MD, since 1977. The facility provides a full range of services, including skilled nursing, rehabilitation, and outpatient therapy, and is home to the only Korean nursing unit in Howard County. With 215 beds across five nursing units and plans for expansion, Lorien Columbia is dedicated to providing exceptional care. Since 2020, it has been recognized as one of the Top Nursing Homes in Maryland by Newsweek Magazine.

Requirements

  • Maryland LPN license, valid and in good standing
  • Every other weekend availability (Saturday and Sunday)

Responsibilities

  • Conduct comprehensive resident assessments and ongoing evaluations
  • Develop, implement, and update individualized care plans
  • Administer medications, treatments, and IV therapies per physician orders
  • Monitor resident conditions and respond to changes in health status
  • Accurately document care, assessments, and outcomes in the EHR
  • Coordinate care with physicians and the interdisciplinary team
  • Communicate effectively with residents and families
  • Supervise and support RN's and CNA/GNAs; delegate tasks appropriately
  • Ensure compliance with state, federal, and CMS long-term care regulations
  • Follow infection control, safety, and quality standards
  • Participate in admissions, discharges, and care plan meetings
  • Promote resident rights, dignity, and person-centered care
  • Respond to emergencies and advocate for resident safety and well-being
