We have an amazing opportunity for a Eastmont - Part-time LPN (Overnight Shift) to join our team! We have an opening for a part-time/PRN Licensed Practical Nurse, LPN, with a 12 hour night position (6 PM to 6 AM) with that includes a great shift differential and skilled nursing incentive! Requirements

Be a graduate of an accredited LPN program
Have an active LPN license in the state, in good standing
Experience in Long-term Care
Have flexibility, able to work the occasional weekend/holiday as needed.
Provide leadership and supervision to nursing team members
Have knowledge of DHS Regulations
Possess superb interpersonal and communication skills and the ability to work well and collaborate with team members and external constituents
Have first aid/CPR certificate per state/local guidelines.
On occasion, may need to push/pull/support up to 50lbs.

Responsibilities

Provide compassionate and effective care and treatments to our residents.
Support the Director of Nursing with assessments, admissions, and other duties as assigned.
Work with a team of professionals to ensure the residents comprehensive needs are met.
Review routines, calendar of events and resident needs
Ensure medications and supplies are ordered, in stock and counted
Ensure all charting and record keeping is accurate and complete
Promote the Mission, Vision and Values of Transforming Age, our Culture of Excellence, in all areas of responsibility.
Perform other duties as assigned.
