About The Position

- The Licensed Practical Nurse (LPN) at Lifestream Behavioral Center is responsible for providing high-quality nursing care to patients within a behavioral health setting. This role involves collaborating with a multidisciplinary team to ensure the delivery of comprehensive healthcare services, while adhering to established protocols and maintaining a supportive and therapeutic environment for patients.

Requirements

Completion of an accredited Licensed Practical Nurse (LPN) program.
0-3 years of experience as a Licensed Practical Nurse, preferably in a behavioral health setting.
Strong understanding of nursing practices and procedures, particularly in mental health care.
Ability to assess patient needs and provide appropriate care and interventions.
Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare team members.
Proficiency in maintaining accurate patient records and documentation.
Ability to work collaboratively within a multidisciplinary team.
Strong organizational skills and attention to detail.
Ability to adapt to changing environments and manage multiple priorities effectively.
Compassionate and patient-centered approach to care.

Nice To Haves

Experience working in a pool or flexible staffing environment is advantageous.

Responsibilities

Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and regulations.
Monitor and document patient vital signs, symptoms, and responses to treatment, ensuring accurate and timely record-keeping.
Assist in the development and implementation of individualized patient care plans, in collaboration with the healthcare team.
Provide direct patient care, including hygiene assistance, wound care, and other nursing interventions as needed.
Educate patients and their families on health maintenance and disease prevention, offering guidance and support to promote patient well-being.
Participate in team meetings and case conferences to discuss patient progress and coordinate care plans.
Maintain a clean, safe, and therapeutic environment for patients, adhering to infection control and safety protocols.
Respond promptly to patient inquiries and concerns, ensuring patient comfort and satisfaction.
Stay updated with current nursing practices and participate in continuing education opportunities to enhance professional development.
Adhere to all organizational policies, procedures, and ethical standards in the delivery of patient care.
