About The Position

- The Licensed Practical Nurse (LPN) at Lifestream Behavioral Center is responsible for providing high-quality nursing care to patients within a behavioral health setting. This role involves collaborating with a multidisciplinary team to ensure the delivery of comprehensive healthcare services, while adhering to established protocols and maintaining a supportive and therapeutic environment for patients.

Requirements

  • Completion of an accredited Licensed Practical Nurse (LPN) program.
  • 0-3 years of experience as a Licensed Practical Nurse, preferably in a behavioral health setting.
  • Strong understanding of nursing practices and procedures, particularly in mental health care.
  • Ability to assess patient needs and provide appropriate care and interventions.
  • Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare team members.
  • Proficiency in maintaining accurate patient records and documentation.
  • Ability to work collaboratively within a multidisciplinary team.
  • Strong organizational skills and attention to detail.
  • Ability to adapt to changing environments and manage multiple priorities effectively.
  • Compassionate and patient-centered approach to care.

Nice To Haves

  • Experience working in a pool or flexible staffing environment is advantageous.

Responsibilities

  • Administer medications and treatments as prescribed by physicians and in accordance with nursing standards and regulations.
  • Monitor and document patient vital signs, symptoms, and responses to treatment, ensuring accurate and timely record-keeping.
  • Assist in the development and implementation of individualized patient care plans, in collaboration with the healthcare team.
  • Provide direct patient care, including hygiene assistance, wound care, and other nursing interventions as needed.
  • Educate patients and their families on health maintenance and disease prevention, offering guidance and support to promote patient well-being.
  • Participate in team meetings and case conferences to discuss patient progress and coordinate care plans.
  • Maintain a clean, safe, and therapeutic environment for patients, adhering to infection control and safety protocols.
  • Respond promptly to patient inquiries and concerns, ensuring patient comfort and satisfaction.
  • Stay updated with current nursing practices and participate in continuing education opportunities to enhance professional development.
  • Adhere to all organizational policies, procedures, and ethical standards in the delivery of patient care.
