Presbyterian Homes & Services – Optage Home and Community Services is looking for an LPN Visit Nurse who feel called to serve older adults in the T win City Metro Area within a caring Christian environment. This is a full-time day position Monday through Friday, as well as participation in an equitable Weekend and Holiday rotation. This position serves as a visit nurse float serving our clients whenere needed throughout the metro area. Key Responsibilities in this role include: Wound care, possibly including wound vac dressing changes, however we can teach you if you don't know how. This would also include prevention strategies (ex: offloading to prevent pressure injuries) Catheter changes and irrigations as ordered/PRN for indwelling and suprapubic catheters Medication teaching, setup, and reconciliation Disease management education and teaching (ex: diabetic education, fingerstick INRs, CHF education with weight and lung sound monitoring, etc.) Being a key member of our interdisciplinary team and keeping everyone involved in each client's care updated on any changes in status In this position you will play a critical role in caring for older adults - not only physically, but emotionally, socially, mentally, and spiritually. Requirements

  • Licensed Practical Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
  • Interest in gerontological nursing, rehabilitative and restorative clinical practices.
  • Demonstrated problem solving skills, dependability, ability to adapt to the scheduling and staffing needs at various PHS communities.
  • Demonstrated compatibility with PHS's mission and operating philosophies.
  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

  • provision and monitoring of high quality clinical care and services to residents consistent with regulations and established clinical best practices
  • providing vital staffing support to our nursing care teams
